Analysis of Loss of Correction in Spinal Fractures treated by Pedicle Screw System



Abstract
In the surgical treatment of spinal fractures. Pedicle Screw Screw System(PSS) became a standard method for correction of deformity. Reduction of fracture and stabilization of spine. We adopted Cotrel-Dubousset pedicle screw system for spinal
fractures
because 1) it is a screw-rod system which enables reduction of sagittal contour of spine easily, and 2) Device for Transverse Traction(DTT) provides sufficient torsional stability. Many authors reported the results of spinal fractures treated by
many
kinds of pedicle screw systems. But there is few report concerning loss of correction. The purpose of this article is to know whether there is significant loss of correction in spinal fractures treated by Cotrel-Dubousset pedicle screw system and
if so,
when it appears and where is the site of loss of correction and which factors influence the loss of correction. The authors analysed 19 patients with spinal fractures and fracture-dislocations treated by Cotrel-Dubousset Instrumentation. The
results
were as follows: Correction loss in body occurs within 1-2 month. While loss in disc develops slowly until 8-12 months period. Burst fracture and old age are considered to be the risk factors of correction loss. Loss of correction can be
prevented
by
anterior augmentation in selected cases.
